not-too-expensive online survey creation tool

Hi all,

Is anyone currently using or aware of a not-too-expensive online survey
creation tool? Files and DB may be deployed in either a Linux/Postgres or
Win/SQL Server environment. Proprietary packages are not preferred, but if
it's reliable, I'll look into it. Of course those kind of packages do a lot
of the work for you, which is appealing!

The surveys might range from 1-50 questions with up to 10,000+ possible
respondents.
