Uploading File Fields

Hello everyone,

I need your assistance on two things. First of all, I am trying to insert a
Browse button for a "File Upload Field" but it's giving me an error. Does
anyone know if this on the "server's administrator's" side of it. I am
using Dreamweaver and I followed all their instructions for creating one.
When I receive by email my test page, there is no attachment.

Also, for our faculty recruitment we'd like to begin doing a similar online
form like Haas School of Business
(http://portal-c.haas.berkeley.edu/AARecruit/index.pl?iid=1937&AdNo=816)
and this is where we got the idea of the file upload field. The question
for this part is, how is your information received that is submitted to
you? Currently any online submission comes to me via email. Does anyone
know how to have the submissions received / dumped into a database? Is this
possible?
