Please submit only the original set of these forms and the related paperwork; a copy is no longer needed.

Lecturer Unit 18: Appointment or Reappointment / Form Instructions (Download the form and open in Acrobat 7 or higher. Opening in the internet browser will cause errors in the calculations on the form)

Change of Appointment: Change to previously approved current or future appointment in your department


Continuing Lecturers

Please submit an original and a copy of these forms and the associated paperwork:

Excellence Review: Review performed in the12th semester. Use APO Excellence Review checksheet [C-4]
(Also requires entry in HRMS case-tracking.)

Continuing Lecturer Merit Increase: Use APO Merit Increase for Continuing Lecturer checksheet [C-6])
(Also requires entry in HRMS case-tracking)

Change of Appointment: Change to previously approved current or future appointment in your department. Submit the original form only; a copy is no longer needed.

  • L&S policy requires an assessment before reappointment to a 7th semester. 
  • The Unit 18 contract requires an assessment for pre-six lecturers if they request consideration for reappointment for the following semester, and if a need exists, see MOU 7a.C.4.
